Credit Card Authorization Form

This form will serve as my electronic signature authorizing American Classic Wedding Car Service, LLC to charge my credit card holding me to all stipulations listed below.

I hereby authorize American Classic Wedding Car Service, LLC to accept my online reservation in which I will use my credit card as a secured electronic payment for the deposit to hold the requested date of my event. Once the payment is successfully processed American Classic Wedding Car Service, LLC will contact me via phone to discuss and finalize my specific contract itinerary details. The completed contract will then be forwarded to me via US mail for my review and signature within 24 hours. I am required to approve, sign and return the contract via US mail. It must be postmarked within 48 hours to finalize the reservation, otherwise this will result in cancellation of the hold on my requested date and result in a deposit forfeiture.

The Balance will be Due in CASH ONLY on the Day of my
Reserved Event at the Time of the First Pickup Location.

NOTE: No Exceptions – Cancellation results in the deposit forfeiture.

I am aware that the members of my party and I must abide by all laws and safety rules. This shall include, no smoking, not using any illegal substances and being of legal drinking age. I also agree to take full responsibility for the actions of myself and all person(s) within my party.

I hereby authorize American Classic Wedding Car Service, LLC
to charge my credit card provided here in for the following fees if necessary:

• I agree that I am financially responsible for any damages done to the vehicle(s) caused by any one passenger in my party, including myself due to carelessness, roughness, horseplay, vomiting or any disregard for the vehicle.
• Customer has the right to cancel the written and signed Event Contract within 24 hours Without Forfeiting the Deposit, after that NO Deposit Will Be Refunded.
• Customer has 60 days prior to Event Date to cancel any or all vehicle(s) reserved.
• Customer will be responsible for entire price of vehicle(s) if cancelled within 60 days of Event Date.

The client is responsible for all attorney fees on behalf of
American Classic Wedding Car Services, LLC in adhering to this contract.
